Responsibilities

Job Summary

Support physicians, nurse practitioners, physician assistants, and/or nurses by taking vitals, assisting in medical examinations and other specific procedures and providing translation services. 

Essential Job Functions

  • Assists participants and supervises with activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, hand and foot nail care and toileting) as assigned or needed.
  • Creates and participates in group(s) and one-on-one activities in accordance with daily activity calendar and as requested by the Recreational Coordinator.
  • Monitor and assist participants with meals at feeding table.
  • Assist throughout the day in maintaining clean and orderly day rooms, kitchen and personal care areas.
  • Accompanies enrollee to medical and other appointments.
  • Perform and assist with laundry, including clothes for participants.
  • Assists the drivers with the loading and unloading of participants from vehicles.
  • Demonstrates dependability through consistent compliance with scheduled work hours.
  • Participates in the activities of the interdisciplinary team as needed and carries out the plan of care as directed.
  • Note changes in participant's physical or emotional health and communicate this information to the appropriate staff person.
  • Maintains participants' medical record and fulfills VCC's PACE charting and reporting requirements.
  • Assists the transportation department with participants' take-home supplies.
  • Participates in and support Quality Improvement initiatives.
  • Follows all VCC's PACE Policies and Procedures and OSHA safety guidelines.
  • Maintains the confidentiality of all company procedures, results, and information about participants, clients or families.
  • Practices standard precautions and follows appropriate Infection Control procedures.
  • Maintains safe working environment. Follows VCC's PACE Safety Policies and Procedures.
  • Perform other duties as directed

Qualifications

Minimum Qualifications

  • High school graduate or equivalent
  • CPR Certification (including BLS Child, Infant, Adult)

Preferred Qualifications

  • Minimum of one years of experience in working with the frail or elderly population preferred.
  • Bilingusl English / Armenian

Required Skills/Knowledge/Abilities

  • Skilled in establishing and maintaining effective working relationships with participants, co-workers, and the public.
  • Basic knowledge in working with frail and elderly to support Activities of Daily Living.
  • Skilled in identifying and recommending problem resolution.
  • Ability to work independently with minimum supervision.
  • Ability to communicate clearly and effectively.
  • Ability to work effectively with cultural, economically, and educationally, diverse populations.

Preferred Skills/Knowledge/Abilities

  • Experience/familiarity with Practice Management and Electronic Medical Records systems, NextGen preferred

Pay Range

  • $23.00 - $26.00 Depending on Experience
