Provide intensive case management and care coordination to program participants and assist in program activities and
administrative support.
• Monitor participants, utilization of care with VCC and other agencies
• Document attempted contacts, client visits and medical assessment information in chart following primary care visit.
• Participate in weekly case conference meetings with interdisciplinary team
• Enroll clients referred by provider into the program
• Monitor participant’s utilization of care with the Clinic and other agencies
• Encourage clients to attend program’s support groups and classes
• Document attempted contacts, actual contacts and summary of services in case notes
• Participate in weekly and monthly staff meetings or as needed
• Complete all monthly activity reports at the end of each month
• May organize and present health education classes
• Coordinate appointments between clients and staff members
• Enhance professional growth and development through participation in educational programs, current literature
review, in-service meetings and workshops
Minimum
High school graduate or equivalent
Five years' case management experience or equivalent combination of education and experience
Bilingual English/Spanish
Valid California driver's license and vehicle insurance; reliable transportation, to travel for meetings and/or to gather information for reporting purposes; minimum two years' driving experience; no more than 2 violations or a single ticket/accident value at more than 1 point on driving record
Availability and willingness to work evening and weekend hours, as required
