Responsibilities

Provide case management, home or field visits, coordinate services for program participants, and assist in program activities and administrative support.

 

• Enroll participants based on program eligibility criteria
• Document case management using the program templates (Service Plan, Case Notes, Referral Log, Contact Log, Consent Forms, and other     forms as directed)
• Monitor the coordination of services and documents additional needs identified for each participant
• Monitor utilization of services and care
• Encourage and monitor that participants complete the educational sessions
• Maintain information on available resources and services in the county and region
• Maintain electronic client files as defined by the program
• Protect client confidentiality following program protocol by utilizing encrypted email and/or fax, and obtain consent to release information       for communication with other agency partners
• Administer surveys, screenings, or assessments as defined by the program

Qualifications

Minimum

  • High school graduate or equivalent
  • Five years’ case management experience or equivalent combination of education and experience
  • Bilingual English/Spanish 
  • Valid California driver’s license and vehicle insurance

Preferred

  • Bachelor's degree in related field with two years’ case management experience

Required Skills/Knowledge/Abilities

  • Excellent verbal and written skills necessary for communication with patients/clients, providers and other staff
  • Ability to interface with all levels of personnel in a professional manner, including people of all social, cultural and ethnic backgrounds and within the constraints of government funded programs
  • Experience/familiarity with computers, business e-mail, communication systems and internet search capabilities and proficient in Microsoft Office products, specifically Word and Excel

Pay Range

  • $23.00 - $25.00
