Responsibilities

Provide patients receiving services through the clinic with access to and linkage with any benefits for which they are eligible and any appropriate services within the clinic or in the community at large.

  • Link with patients who are currently uninsured, or significantly under-insured to assist in determining eligibility for, and accessing and navigating health plan benefits
  • Assist with enrollment or link with a certified enrollment counselor to enroll patient in the appropriate insurance plan and behavioral health benefits through the insurance plan
  • Link with partnering County-funded or contracted mental health clinics or substance use disorder treatment providers for adults in need of specialty mental health or Substance Use Disorder (SUD) treatment
  • Assist with linkage to mental health, substance use disorder, and/or Medicated Assisted Treatment (MAT) prescribers within the health center for adults in local emergency department; attend transition visit appointments at collaborating county-contracted specialty mental health and substance use disorder agencies for patients that will be transitioning to the health center to receive care in the areas of behavioral health, substance abuse disorders, or primary care
  • Provide availability for patient-centered support services during normal business hours within one business day of request
  • Assist patients from partnering Federally Qualified Health Centers (FQHCs) in the Behavioral Health Primary Care Integration Services (BHPCIS) initiative with their behavioral health and primary care needs by drawing upon their personal experiences with behavioral health challenges, access to and receipt of care and maintenance through their health journey
  • Conduct handoffs of patients to certified enrollment counselors at partnering FQHCs and help patients organize required paperwork as needed
  • Provide guidance to patients on a one-on-one and/or group basis
  • Provide justice-impacted patients receiving services through the clinic with access to and linkage with any benefits for which they are eligible and any appropriate services within the clinic or in the community at large

Qualifications

Minimum

  • High school graduate or equivalent
  • Five years’ experience in mental health, recovery, or case management, or equivalent combination of education and experience
  • Valid California driver’s license and proof of auto insurance
  • Clean driving record for a minimum period of three years prior to hire; must maintain clean driving record throughout employment
  • Availability and willingness to work evening and weekend hours, as required

Preferred Qualifications

  • State and/or National Certified Peer Specialist (NCPS) Certification
  • Experience working with community health centers or underserved populations

Required Skills/Knowledge/Abilities

  • Ability to present information effectively both orally and in writing to individuals and groups from various cultural, ethnic and educational backgrounds
  • Ability to interact with all VCC personnel and the public in a professional manner
  • Experience/familiarity with computers, business e-mail, communication systems, data entry skills and internet search capabilities; basic knowledge in Microsoft Office products, specifically Word and Excel
  • Ability to work independently, take initiative and be proactive

Preferred Skills/Knowledge/Abilities

  • Experience/familiarity with Practice Management and Electronic Medical Records systems, NextGen preferred
  • Experience/familiarity with computers, Microsoft Office products, business e-mail communication systems and internet search capabilities

Pay Range

  • $23.00 - $25.00
