Responsibilities

Provide HIV medical case management to program participants and assist in program activities and administrative support.

  • Enroll clients into the program
  • Provide intensive case management assessment and care coordination to clients
  • Monitor participant’s utilization of care with VCC and other agencies
  • Coordinate appointments between clients and staff

Qualifications

Minimum

  • High school graduate or equivalent

  • Five years' case management experience or equivalent combination of education and experience

  • Bilingual English/Spanish

  • Valid California driver's license and vehicle insurance; reliable transportation, to travel for meetings and/or to gather information for reporting purposes; minimum two years' driving experience; no more than 2 violations or a single ticket/accident value at more than 1 point on driving record

  • Availability and willingness to work evening and weekend hours, as required

Required Skills/Knowledge/Abilities

  • Knowledge of co-occuring conditions
  • Effective written/verbal communication skills
  • Familiarity with local HIV/AIDS and non-HIV specific resources for health and social service needs
  • Familiarity with health and social service needs of target populations served
  • Basic computer knowledge, internet use, MS Office and data entry skills

Pay Range

  • $22.00 - $24.00
