Responsibilities

While working with patients of different ages and health related issues, addresses complex issues surrounding food and nutrition. Responsible for creating nutritional programs based on the individuals’ specific medical and health needs. Ensure the patient’s personal and cultural preferences are incorporated into a plan that will ultimately improve their overall health. Responsible for reinforcing the proposed plans through education of the patient and their families on how to properly follow the dietary guidelines. Provide follow up on patients’ nutritional progress, making adjustments to the dietary plan as needed.   • Conduct nutrition consultations
  • Advise patients on setting specific nutritional goals
  • Provide patients with educational facts on nutrition to improve their health
  • Evaluate patient lifestyle, nutritional and health needs
  • Establish short and long-term goals for each patient, incorporating their personal and cultural preferences into the
    plan of care
  • Develop a thorough and cost-efficient meal plan that includes healthy foods and meets nutrition goals
  • Work with Diabetes Care Team to track patient progress and adjust meal plans as needed
  • Organize educational materials/classes that include recipes and meal ideas
  • Ensure dietary guidelines are in sync with “Patient Centered Medical Home” model
  • Work with patients to make changes in eating habits to help prevent or mitigate chronic illnesses such as diabetes,
    heart disease, osteoporosis, kidney disease, high blood pressure and cancer

Qualifications

Minimum

  • Bachelor's degree in related field
  • Registered Dietician certification 
  • Minimum  three years' experience in nutritional counseling
  • Bilingual English/Spanish
  • Valid California driver’s license, current vehicle insurance and reliable transportation

Preferred

  • Lactation Educator or Consultant certification and/or training

Required Skills/Knowledge/ Abilities

  • Experience/familiarity with computers, Microsoft Office products (Word/Excel), business e-mail communication systems and internet search capabilities

Salary:

  • $37.00 - $44.00 per hour DOE
