Overview

Vista Community Clinic is a private, non-profit, multi-specialty outpatient clinic providing care in a comprehensive, high quality setting. Responsibilities

  • Enroll mentees into the program and conduct initial status assessments
  • Monitor and track mentee's progress on a monthly basis and share with Lead Mentor
  • Work with mentees and families to assist them in meeting their needs
  • Identify assets and experiences outside of the mentee's neighborhood to broaden mentee's world view and sense of possibility, and accompany mentee to these places
  • Provide intensive case management assessment and care coordination to clients
  • Generate referrals to VCC and other agencies for services/resources and monitor follow-through
  • Provide intensive support, advice, and guidance during twice weekly group sessions, outside the sessions, and as otherwise specified in the mentee's personal case plan
  • Work with HHSA personnel to access services applicable to mentees and their families
  • Participate in weekly staff meetings with program team
  • Lead interactive journaling, participate in other group/social activities with mentees, and provide one-on-one mentorship

Qualifications

Minimum

  • High school graduate or equivalent with five years' case management experience.
  • Valid California driver’s license, current vehicle insurance and reliable transportation
  • Availability and willingness to work evening and weekend hours, as required.
  • Should be community member with relevant life experience and social capital that gives authority to challenge the norms and change the thinking of those youth with whom they mentor.
  • Have current or prior criminal justice involvement with a record of success and be positively engaged in change
  • Must be at least 18 years of age
  • Undergo background checks conducted by Probation to comply with Board Policy C-28-Criminal Background Check History and be subject to Probation approval 

Preferred

  • Associates degree plus four years' case management experience or Bachelor's degree in related field with two years' case management experience
  • Knowledge of local social service landscape
  • Have connections with mentee (e.g. similar neighborhood, school, shared family or friends)
  • Bilingual English/Spanish

Required Skills/Knowledge/Abilities 

  • Ability to present information effectively verbally and in writing to individuals and groups from various cultural, ethnic and educational backgrounds
  • Ability to work independently and under time constraints
  • Ability to handle confidential materials and information in a professional manner

Pay Range

$21.00
