Responsibilities

Provide support to individuals at risk for HIV by helping them access and stay on PrEP (Pre-Exposure Prophylaxis) and PEP (Post-Exposure Prophylaxis) services. Conduct HIV/HCV (Hepatitis C Virus) rapid testing, outreach, education, and referrals. Help patients move through the health care system, remove barriers to care, and connect them to needed services. Provide related services as required by program guidelines and directed by the Manager/Supervisor.

• Help patients start and stay on PrEP or access PEP when needed
• Schedule appointments across clinics and mobile medical units
• Coordinate care with clinical staff and other team members
• Assist patients with health insurance enrollment and renewal
• Work with pharmacies on patient assistance programs (PAP)
• Follow up with patients to confirm medication pick-up and address issues with meds or insurance
• Answer questions from patients, staff, and community members about PrEP/PEP
• Maintain an active caseload and provide ongoing follow-up to support patient engagement in care
• Refer patients to other services based on their needs (health education, harm reduction, medical care, etc.)
• Collect, track, document, and enter participant and program data in required systems to support funding requirements, program evaluation, quality improvement, and internal and external program reporting.
• Monitor progress toward program objectives and recommend updates or improvements as needed.
• Conduct HIV and HCV rapid testing in clinic and community settings
• Provide education on HIV prevention, including PrEP and PEP and STIs
• Conduct outreach in person and online, including in non-traditional settings (encampments, shelters, community sites, social and online networking sites)
• Provide presentations and outreach education on HIV prevention
• Maintain accurate and timely documentation while protecting patient confidentiality
• Prepare and distribute outreach materials and prevention supplies as needed
• Document services, outreach, and patient interactions in required systems
• Participate in team meetings and program activities
• Maintain professional relationships with patients, staff, and community partners
• Travel to community sites and transport supplies as needed
• Attend meetings, trainings, and community events as needed
• Enhance professional growt

Qualifications

Minimum Qualifications


• High school graduate or equivalent
• Minimum of one year experience in community education
• Bilingual English/Spanish
• Valid California driver’s license and vehicle insurance; reliable transportation, to travel for meetings and/or to gather information for reporting purposes; minimum two years’ driving experience; no more than 2 violations or a single ticket/accident value at more than 1 point on driving record
• Ability and willingness to work flexible hours, including evenings and weekends

 

Preferred Qualifications


• Associate or bachelor’s degree in related field with work or volunteer experience in community education

• Ability to present information accurately and effectively both verbally and in writing to individuals and groups in both English and Spanish
• Ability to create a comfortable, supportive learning environment
• Ability to interact with all VCC personnel and the public in a professional manner

Pay Range

  • $22.50 - $23.00 Depending On Experience
