    Responsibilities

    Under the supervision of the prenatal program manager, provide support to the program manager, CPSP providers and referral coordinators with case management, appointment setting, and various clerical duties. Receive patients after provider visits and complete orders for follow-up appointments, labs and referrals as directed. Insure that all patients have follow-up appointments with CPSP staff. Assist patients in completing video interviewing for Medi-Cal benefits. Act as a liaison between VCC and TCMC for delivering moms by entering outcomes, birth histories and scheduling appointments for follow-up at the clinic. Rotate through all prenatal services and locations as needed.

    • Assist with the preparation of ultrasound appointments
    • Locate outside reports and lab results
    • Electronically send records to TCMC at 24 wk visit and updates at 36 – 40 wk visits
    • Fax records to TCMC as requested
    • Provide assistance to the prenatal sites
    • Complete the GDM status report
    • Complete monthly reports as directed by program manager
    • Assist the CPSP providers in scheduling appointments and making reminder calls, as needed
    • Make reminder calls for intakes
    • Provide assistance as needed to the prenatal program manager, referral coordinators and CPSP providers
    • Assist with quality assurance and program safety
    • Assist with translating as needed
    • Receive patients after their provider visits and carry out provider orders as directed
    • Instruct clients on the documents necessary for the Medi-Cal eligibility process
    • Provide overview of Medi-Cal program and reinforces Presumptive eligibility coverage and requirements
    • Review the schedule and call clients to remind about appointment
    • Follow up and reschedule clients who fail to keep their appointment
    • Schedule the client with the DSS worker via video interviewing
    • Assist with the downloading and of encrypted documents during video interviewing
    • Fax/ mail completed forms to DSS as directed
    • Fax schedules to DSS worker as directed
    • Follow up and reschedule clients who fail to keep their DSS appointment
    • Perform tracking activities such as highlighting the Presumptive Eligibility log tracking Medi-Cal screener/DSW appointments that were kept
    • Complete daily/weekly and monthly reports
    • Provide a link between the prenatal department staff and DSS
    • Visit delivering patients at TCMC as directed
    • Register newborns in Next Gen EPM
    • Enter delivery outcome in mother’s prenatal record , newborn’s birth history in Next Gen EMR
    • Schedule newborn appointments at clinic per protocol
    • Schedule mother’s follow up post partum appointments (Provider, CPSP Provider, Nutritionist and Social Worker) at clinic per protocol
    • Provide mothers with all appointments on the same day as hospital visit
    • Attend meetings as required
    • Enhance professional growth and development through participation in educational programs, current literature review, in-service meetings and workshops
    • Support the vision, mission and goals, and demonstrate a commitment to the values, of the organization
    • Perform other duties as directed

    Qualifications

    Minimum Qualifications

    • High school graduate or equivalent
    • One year related experience
    • Bilingual English/Spanish
    • Valid California driver’s license, current vehicle insurance, and reliable transportation to travel between sites for meetings and/or to gather information for reporting purposes

    Preferred Qualifications

    • Experience/familiarity with Practice Management and Electronic Medical Records systems, NextGen preferred

    Required Skills/Knowledge/Abilities

    • Experience/familiarity with computers, Microsoft Office products, business e-mail communication systems and internet search capabilities
    • Ability to present information both orally and in writing and possess good organizational skills
    • Ability to work independently with minimal supervision
    • Ability and willingness to meet the organization’s attendance and dress code policiesAbility to handle confidential materials and information in a professional manner
    • Excellent customer service skills and commitment to providing the highest level of customer satisfaction
    • Familiar with operation of telephone, fax and copier equipment
    • Good verbal and written skills necessary for communication with patients/clients, providers and other staff
