    Vista Community Clinic is a private, non-profit medical, dental, optometry, chiropracty, behavioral and social services center providing care in a comprehensive, high quality setting. We provide the highest quality services in seven clinics located in San Diego, Orange and Riverside counties.     • Enroll clients into the program and conduct initial status assessments
    • Provide intensive case management assessment and care coordination to clients
    • Generate referrals to VCC and other agencies for services/resources and monitor follow-through
    • Encourage clients to attend program’s workshops and classes
    • Document contacts with case manager and broader program participation
    • Participate in weekly staff meetings with program team
    • Complete all monthly activity reports at the end of each month and work with Data Specialist and evaluation consultants
    • Conduct pre and post evaluations



    • High school graduate or equivalent with five years case management experience
    • Valid California driver’s license, current vehicle insurance, and/or reliable transportation to travel between sites for meetings with clients and partner agencies
    • Availability and willingness to work evening and weekend hours, as required


    • Associates degree plus four years case management experience or Bachelor's degree in related field with two years case management experience
    • Knowledge of local social service landscape
    • Bilingual English/Spanish

    Required Skills/Knowledge/Abilities 

    • Good verbal and written skills necessary for communication with clients and staff
    • Ability to present information effectively orally and in writing to individuals and groups from various cultural, ethnic and educational backgrounds
    • Ability to work independently and under time constraints
    • Ability to handle confidential materials and information in a professional manner
