  • Overview

    Manage the care coordination team/panels with a focus on population health to improve health outcomes of VCC patients.
    Supervises.  Care Coordinators and Schedulers

    Responsibilities

     Primary duty consists of:

    • Hiring, firing, promoting, training, evaluating and/or directing the work of designated staff
    • Pursue innovative ideas to improve the health of populations, especially as they relate to the Care Coordinator Panels
    • Contribute to the Quality Improvement team to help with Population Health efforts
    • Assist in the validation of patient panels (for target populations)
    • Improve health outcomes and reduce health disparities for Vista Community Clinic patients with and without chronic diseases
    • Ensure the staff are following the initiative of VCC to educate and empower patients to live healthier
    • Participate in program planning
    • Act as a resource person for all staff and patients; resolve patient and staff complaints
    • Coordinate with billing in the management and completion of Comprehensive and Annual Wellness
    • Assist in developing Transition of Care Strategies
    • Assist in the collaboration between nurses and care coordination to care for medically complex patients
    • Participate in Clinical Outcomes Committee, Quality and Risk Management, and other relevant committees as they pertain to population health and quality improvement
    • Enhance professional growth and development through participation in educational programs, current literature review, in-service meetings and workshops
    • Support the vision, mission and goals

    Qualifications

    Minimum

     

    • Licensed as Registered Nurse in the State of California
    • Minimum five years’ RN experience

    Preferred Qualifications

    •  Two years’ Case Management experience; in a hospital and/or community setting
