M, W, F - 8:00 a.m. to 4:30 p.m., Tues. 10:00 a.m. to 6:30 p.m., Thurs., 11:00 a.m. to 7:30 p.m., Sat. as needed.

Overview

Provide intensive case management and home visitation to program participants and assist in program activities and administrative support under general supervision. Coordinate appointments between clients and staff members.

Responsibilities

 

  • Enroll clients referred by provider into the program
  • Monitor participant’s utilization of care with the Clinic and other agencies
  • Document attempted contacts, actual contacts and summary of services in case notes
  • May organize and present health education classes

Qualifications

Minimum

  • Must have Bachelor's degree 
  • Valid California driver’s license, current vehicle insurance, and reliable transportation
  • Bilingual English/Spanish 
  • Availability and willingness to work evening and weekend hours, as required

Required Skills/Knowledge/Abilities

  • Ability to communicate effectively, verbally and in writing, to individuals and groups from varying cultural, ethnic, social and educational backgrounds, and within the constraints of government funded programs.
  • Proficient in MS Word, Excel, PowerPoint and Outlook
