M, W, F - 8:00 a.m. to 4:30 p.m., Tues. 10:00 a.m. to 6:30 p.m., Thurs., 11:00 a.m. to 7:30 p.m., Sat. as needed.
Provide intensive case management and home visitation to program participants and assist in program activities and administrative support under general supervision. Coordinate appointments between clients and staff members.
Enroll clients referred by provider into the program
Monitor participant’s utilization of care with the Clinic and other agencies
Document attempted contacts, actual contacts and summary of services in case notes
May organize and present health education classes
Must have Bachelor's degree
Valid California driver’s license, current vehicle insurance, and reliable transportation
Availability and willingness to work evening and weekend hours, as required
Ability to communicate effectively, verbally and in writing, to individuals and groups from varying cultural, ethnic, social and educational backgrounds, and within the constraints of government funded programs.
Proficient in MS Word, Excel, PowerPoint and Outlook
