Overview

Assist patients in completing eligibility and application packets. Provide direct information, education, and referrals to individuals and groups in target population as required by program and/or as directed. Provide case management and prenatal education to individuals and groups of prenatal patients receiving services at Vista Community Clinic; coordinate and assist in planning, developing, and implementing program strategies; evaluate, develop and maintain health education materials and methods; may provide training to other health education staff.

Responsibilities

  • Follow strict guidelines for eligibility processing
  • Review the schedule and call clients to remind about appointment; follow up and reschedule clients who miss appointment
  • Assist clients in the completion of the application package
  • Assist in planning, developing and implementing program strategies
  • Evaluate, develop and assist in maintaining health education materials
  • Maintain documentation and logs for program
  • Complete and maintain assessments for health education, nutrition and psychosocial CPSP components
  • Provide case management services to clients including coordination with client, family members, client support systems and members of the interdisciplinary team
  • Process necessary referrals as identified in the assessment and as recommended by providers

Qualifications

Minimum:

  • High School diploma or equivalent
  • Minimum of two years’ experience in community education and/or direct patient services
  • Bilingual English/Spanish
  • Availability and willingness to work flexible hours, including evenings and weekends
  • Valid California driver’s license, current vehicle insurance, and reliable transportation to travel between sites for meetings and/or to gather information for reporting purposes
