Overview

Seeking Comprehensive Perinatal Health Worker who will provide case management and prenatal education to individuals and groups of prenatal patients receiving services at Vista Community Clinic. 

 

Vista Community Clinic is a non-profit organization that is working to advance community health and hope by providing access to premier health services and education for those who need it most.

Responsibilities

  • Utilize OB triage protocol to conduct triage on patients
  • Conduct patient “checkout” to ensure all required services were received and that all follow-up appointments and referrals are scheduled
  • Complete and maintain assessments for health education, nutrition and psychosocial CPSP components
  • Provide case management services to clients including coordination with client, family members, client support systems and members of the interdisciplinary team

 

Qualifications

Minimum

  • High school graduate or equivalent
  • Minimum one year maternal and child health experience, including direct patient services
  • Bilingual English/Spanish
  • Valid California driver’s license, current vehicle insurance and reliable transportation

Preferred

  • Medical Assistance Certificate

Required Skills/Knowledge/Abilities

  • Experience with a Medi-Cal program in an outpatient setting
